If my salary is above 1.5k per month that mean i am not covered by employment act and all the terms and conditions should follow the agreement letter. If that so, can the employer state in the appointment letter that maternity for female employee is only one month?

It can be that way if you did not at the time interview negotiate all the terms and conditions properly. Look after your own interests.
