I was hospitalised for 3 weeks. Then i was discharge from hospital and given another 2 weeks for rest at home. Does the 2 weeks consider as hospitalisation leave or sick leave?

Usually yes. Do refer to your appointment letter. If you are given paid sick leaves during probation by your employer, it is a grace to you.

Sick leave is given pro rated, but what about hospitalization? Shall it be pro-rated as well?

For hospitalization, it is a matter of whether one is entitled or not. If he/she is entitled, usually no pro-rated will be asked.

Sec 60F (bb) of sixty days in the aggregate in each calendar year if hospitalisation is necessary, as may be certified by such registered medical practitioner or medical officer:
Provided that the total number of days of paid sick leave in a calender year which an employee is entitled to under this section shall be sixty days in the aggregate;
And provided further that if an employee is certified by such registered medical practitioner or medical officer to be ill enough to need to be hospitalised for any reason whatsoever, the employee shall be deemed to be hospitalised for the purposes of this section.
