staff working attendance for july 2008 only 14 days, how to calculate his salary? (not report to work without inform office and apply any leave). Salary for him need to deduct EPF and Socso? Pls advise. Thank you.

If you don't want him to pay indemnity to the company, his salary will be (salary/31)x14.5 You of course need to pay EPF and SOCSO.

staff taken unpaid leave from Jan - July 2008 total 57 days.He still entitled to claim annual leave for year 2008?

Actually, in a case like this, you just don't pay him his 14.5 days salary. Withhold his salary as indemnity because he has broken the contract of service for not turned up for work. Let him go to the Labour Office and complain.
